New Patients at Carriage Hill Family Care

Carriage Hill Family Care is accepting new patients.

We provide primary care for children and adults in Paragould and the surrounding communities. To schedule a new patient appointment, call our office at (870) 573-2200.


Before Your First Visit

Please arrive at least 30 minutes before your scheduled appointment to allow time for registration and any remaining paperwork.

Please bring:

  • Photo ID
  • Current insurance card
  • An up-to-date medication list, including doses
  • Any information you would like your provider to review regarding your medical history 


New Patient Forms

Completing your paperwork before your visit can help reduce check-in time. You may download and complete our New Patient Packet before your appointment.

You may bring the completed forms with you to your appointment.


Medical Records

Records from your previous healthcare providers are helpful but are not required before your first appointment. If you would like records sent to Carriage Hill Family Care, you may complete a medical records release form.

What Patients May Owe
