Carriage Hill Family Care, PLC is an independent primary care practice serving children and adults in Paragould and throughout Northeast Arkansas.


Our medical and mental health professionals provide preventive care, evaluation and treatment of acute illnesses, chronic disease management, behavioral health services, and ongoing primary care. Dr. Vincent Lee is a board-certified family medicine physician and founder of Carriage Hill Family Care, PLC in Paragould, Arkansas. He provides comprehensive primary care for patients of all ages, with an emphasis on preventive medicine, chronic disease management, and patient education. 


Raised in Forrest City and deeply rooted in Northeast Arkansas, Dr. Lee is committed to improving the health of his community through accessible, personalized, and evidence-based care. 


In addition to his clinical practice, Dr. Lee serves as the County Health Officer for Greene County, providing leadership in public health initiatives, emergency preparedness, and community outreach. He is an active member of the American Academy of Family Physicians and the Arkansas Medical Society. 


Dr. Lee has completed advanced training in Adult Behavioral Health in Primary Care and Practical Behavior Management in Pediatric Primary Care through The REACH Institute. This training supports evidence-based diagnosis and treatment of anxiety, depression, ADHD, substance use disorders, and related conditions within the primary care setting. 


Before founding Carriage Hill Family Care, Dr. Lee co-owned and directed Paragould Family Care and contributed to wound care services at St. Bernards Wound Healing Center. His clinical experience also includes occupational medicine, DOT examinations, Medical Review Officer services, and the care of patients with complex acute and chronic medical conditions.

Cecil Massey, APRN, has more than two decades of nursing and advanced practice experience and has been a provider at Carriage Hill Family Care since the practice opened in 2022. He provides comprehensive primary care for patients of all ages, including preventive care, acute illness treatment, and management of chronic medical conditions.


A U.S. Navy veteran, Cecil served from 1985 to 1991, including active duty during Operation Desert Storm. His military service provided a strong foundation in leadership, discipline, and service that continues to inform his approach to patient care.


Cecil is also a certified DOT Medical Examiner and performs Department of Transportation physical examinations for commercial drivers. He has extensive experience with occupational medicine and workers' compensation cases, including workplace injury evaluation, treatment, care coordination, and return-to-work planning.


He is a member of the American Nurses Association, Arkansas Nurse Practitioner Association, Sigma Theta Tau, and Phi Kappa Phi Honor Society. His clinical experience spans primary care, occupational medicine, chronic disease management, and the evaluation and treatment of acute medical concerns.

Teresa Gonzalez, APRN, has been a provider at Carriage Hill Family Care since the practice opened in 2022. Her nursing career began in 1987, giving her decades of clinical experience in both nursing and advanced practice care.


Before becoming a family nurse practitioner, Teresa worked in multiple clinical and leadership roles at Arkansas Methodist Medical Center, including intensive care, emergency care, medical-surgical nursing, and house supervision. This broad background provides a strong foundation for caring for patients with both acute and chronic medical needs.


Teresa provides comprehensive primary care for patients across the lifespan and has particular experience in women's health and family medicine. Her clinical focus includes preventive care, wellness visits, chronic disease management, acute illness treatment, and women's health concerns throughout adulthood.


She is a member of the Arkansas Nurses Association and the American Nurses Association and remains active in continuing professional education. Her extensive nursing background and advanced practice experience support a practical, patient-centered approach to primary care.

Latoya Coward, APRN, has more than a decade of experience as a family nurse practitioner and provides comprehensive primary care for children and adults. She has extensive experience in pediatric care and enjoys developing long-term relationships with children and their families as they grow.


Her clinical interests include well-child and preventive care, evaluation and treatment of acute illnesses, chronic disease management, and supporting healthy growth and development from childhood through adulthood.


Latoya is a member of the American Association of Nurse Practitioners and remains active in continuing professional education.


A Paragould resident since 2012, Latoya has also been active in the local community. She previously served as President of the Junior Auxiliary of Paragould and remains a lifetime member of the organization.


Latoya has been a provider at Carriage Hill Family Care since the practice opened in 2022. Her experience in pediatric and family medicine supports comprehensive primary care for patients and families throughout the community.

Lauren Willmer, APRN, has been a provider at Carriage Hill Family Care since the practice opened in 2022. She earned her Doctor of Nursing Practice with Family Nurse Practitioner training and provides comprehensive primary care for children and adults.


Lauren has extensive experience caring for pediatric patients and enjoys providing care for children as well as entire families. Her pediatric experience includes well-child and preventive care, evaluation and treatment of acute illnesses, and support for healthy growth and development throughout childhood and adolescence.


Her clinical interests also include adult preventive care, chronic disease management, acute illness treatment, and long-term primary care across the lifespan.


Lauren's broad family practice experience allows her to care for patients through different stages of life while developing long-term relationships with individuals and families in the community.

Allyssa Beall, APRN, joined Carriage Hill Family Care in 2022. Before becoming a family nurse practitioner, she spent five years working as an intensive care unit nurse. She later gained additional experience in urgent care and family medicine in Paragould.


Her critical care and urgent care background provides experience in evaluating acute and complex medical concerns, while her family medicine practice includes preventive care, chronic disease management, acute illness treatment, and comprehensive primary care for patients across the lifespan.


In addition to primary care, Allyssa is certified in BOTOX Cosmetic treatment and provides aesthetic services for appropriate patients. She combines her clinical training with an individualized approach to treatment planning and patient safety.


Allyssa is a member of the American Association of Nurse Practitioners and the Junior Auxiliary of Paragould. Her background in critical care, urgent care, family medicine, and aesthetic treatment brings a broad range of clinical experience to Carriage Hill Family Care.

Diane Underwood, APRN, joined Carriage Hill Family Care in 2023 and brings more than a decade of nursing experience to her role as a family nurse practitioner. Her previous clinical experience includes intensive care and gastrointestinal lab nursing, providing a strong foundation for evaluating both acute and chronic medical concerns.


Her nursing education progressed from practical nursing to registered nursing and advanced practice. She earned her Master of Nursing Science in Family Practice from the University of Arkansas for Medical Sciences in 2021, graduating with honors.


Diane provides comprehensive primary care for patients across the lifespan, including preventive care, acute illness treatment, chronic disease management, and routine health maintenance. As a Certified Nurse Educator, she places particular emphasis on helping patients and families understand their medical conditions, treatment options, medications, and preventive health needs.


Her clinical approach emphasizes long-term relationships with patients and families, shared decision-making, and practical education that helps patients take an active role in their healthcare.


Diane is a member of Phi Theta Kappa and the Arkansas Nurse Practitioners Association and remains active in continuing professional education.

Laura Needham-Puckett, LPC-S, joined Carriage Hill Family Care in 2024 and brings more than 20 years of experience providing mental health counseling to children, adolescents, and adults. Her clinical background includes individual and family counseling, trauma-focused treatment, play therapy, behavioral health, and psychological assessment.


Laura works with patients experiencing anxiety, depression, trauma and post-traumatic stress disorder (PTSD), life transitions, caregiver stress, parenting challenges, behavioral and emotional concerns, and family relationship difficulties. Treatment is individualized according to each patient's age, circumstances, needs, and goals.


As a Registered Play Therapist Supervisor, Laura has advanced training in working with children through developmentally appropriate play therapy and parent consultation. She also incorporates person-centered therapy, cognitive behavioral interventions, Adlerian principles, and trauma-focused approaches into her work when appropriate.


Laura's experience in psychological assessment and behavioral health supports collaboration with the medical providers at Carriage Hill Family Care, allowing mental and physical health needs to be addressed as part of a coordinated approach to patient care.
